Oculoplasty is the branch of ophthalmology concerned with the structures around the eye — the eyelids, the tear drainage system, the orbit and the socket. The work is both functional and cosmetic, and often both at once: a drooping eyelid that blocks vision is a functional problem whose correction also changes how someone looks.

Ptosis — a drooping upper eyelid

The eyelid sits lower than it should, sometimes covering part of the pupil. It may be present from birth or develop with age as the muscle attachment stretches. In children, ptosis that obstructs the visual axis needs timely attention because it can cause amblyopia (lazy eye). In adults, the usual complaints are a tired appearance, a restricted upper field of vision, and having to lift the chin or raise the brow to see.

Entropion and ectropion

In entropion the eyelid turns inward and the lashes rub on the cornea, causing irritation, watering and, if untreated, corneal damage. In ectropion the lid turns outward, leaving the eye exposed and watering. Both are usually age-related and both are corrected surgically.

Watering eyes and blocked tear ducts

Persistent watering is often a drainage problem rather than excess tear production. When the nasolacrimal duct is blocked, tears cannot drain into the nose. In adults, the usual treatment is dacryocystorhinostomy (DCR), which creates a new drainage passage; it can be done externally or endoscopically. In infants, a blocked duct frequently resolves on its own during the first year with massage, and probing is considered if it does not.

Eyelid lumps: chalazion and stye

A chalazion is a blocked oil gland forming a firm, usually painless lump; a stye is an acute, tender infection at the lash line. Many settle with warm compresses, lid hygiene and medication. A chalazion that persists is treated with a small incision and curettage. Any eyelid lump that is growing, ulcerating, bleeding, distorting the lid margin or causing lash loss should be examined promptly rather than treated as a simple chalazion.

Thyroid eye disease

Thyroid dysfunction can cause the eyes to protrude, the lids to retract, and the eyes to feel dry or appear staring. Management is coordinated with the treating physician, and surgery is generally considered once the condition is stable.

Trauma and reconstruction

Eyelid lacerations, particularly those involving the lid margin or the tear drainage canaliculi, need careful repair to restore both function and appearance. Orbital fractures are assessed with imaging and managed accordingly.

What to expect from surgery

Most eyelid procedures are done under local anaesthesia as day care, and take roughly 30–60 minutes depending on the operation. Bruising and swelling for one to two weeks are normal, and cold compresses in the first 48 hours help. Fine sutures are usually removed after about a week unless absorbable sutures are used. The final appearance settles over several weeks to a few months as the scar matures.

Eyelid surgery is precise work in a small, highly visible area with a wide range of normal anatomy. Some asymmetry can persist, and occasionally an adjustment is needed. That is discussed with you before you consent, and it is one reason photographs are taken beforehand.

Questions patients ask

Is a drooping eyelid only a cosmetic problem?

Often not. A lid that covers part of the pupil restricts the upper field of vision, and in children it can cause amblyopia. Many people also unconsciously raise the brow or tilt the chin to compensate, which causes headaches and neck strain.

My eyes water constantly. Does that mean I need surgery?

Not necessarily. Watering can come from dry eye, lid malposition, infection or a drainage blockage, and the treatment differs completely. The cause is established first with an examination and, where needed, syringing of the tear passage.

My baby's eye waters and sticks. Is that serious?

A blocked tear duct is common in infants and most resolve within the first year with massage and hygiene. It should still be examined, because other causes need excluding, and probing is considered if it persists.

Will there be a visible scar?

Incisions are placed in natural skin creases or on the inner surface of the lid wherever possible. Scars usually fade considerably over several months, though healing varies between individuals.

How long is recovery from eyelid surgery?

Bruising and swelling are noticeable for one to two weeks. Most people feel presentable after about two weeks, though the final result settles over a few months.

Is a lump on my eyelid dangerous?

Most are benign chalazia or styes. A lump that grows, bleeds, ulcerates, distorts the lid margin or causes lash loss needs prompt examination, as eyelid cancers do occur and are very treatable when caught early.
