Micro-incision cataract surgery, or MICS, is a modern approach to removing a cataract through a very small opening in the eye β€” typically under about 2 millimetres. The small incision is designed to be self-sealing, so in most cases no stitches are needed and the eye is left as undisturbed as possible.

What happens during MICS?

A cataract is the clouding of the eye's natural lens. In MICS, the cloudy lens is gently broken up and removed using ultrasound energy (phacoemulsification) passed through a fine probe. A clear, foldable artificial lens β€” the intraocular lens, or IOL β€” is then inserted through the same tiny opening and unfolds into position. Because the lens is folded, it fits through a much smaller incision than older techniques required.

Why a smaller incision can help

πŸ’‘ Is it right for everyone? Suitability depends on your eye, the type of cataract and your overall eye health. The best way to know is a detailed examination and measurement of the eye before surgery.

Before the day of surgery

Careful planning is a large part of a smooth result. This usually includes:

  1. A complete eye examination.
  2. Precise measurements of the eye (biometry) to select the right lens power.
  3. A discussion of lens options suited to your eyes and daily needs.
  4. Simple instructions on eye drops and what to expect on the day.

What to expect afterwards

MICS is usually carried out with anaesthetic drops, so the eye is numb and you remain awake but comfortable. Afterwards you will use prescribed eye drops for a few weeks and attend a short follow-up. Most people are advised to avoid rubbing the eye, swimming and dusty environments for a little while as it heals. Your surgeon will give you guidance tailored to your eye.
