Liver elastography is a simple, painless ultrasound scan that measures how stiff your liver is. Because scarring (fibrosis) makes liver tissue firmer, measuring stiffness gives your doctor a useful, non-invasive picture of liver health β often without the need for a liver biopsy.
What is liver elastography?
A healthy liver is soft. When the liver is inflamed or scarred over time, it becomes stiffer. Shear-wave elastography uses a standard ultrasound probe to send a gentle pulse into the liver and measure how quickly it travels through the tissue. The result is reported as a stiffness value in kilopascals (kPa). It is performed on the same visit as a regular ultrasound and feels exactly the same to you.
Why might it be advised?
- Fatty liver β to check whether fat build-up has begun to cause scarring.
- Hepatitis B or C β to assess and monitor the liver over time.
- Raised liver enzymes on a blood test with no clear cause.
- Regular alcohol use or other long-standing liver conditions.
- Monitoring β to track whether the liver is stable, improving or changing.
π‘ Good to know: elastography assesses stiffness, while a separate measurement can assess fat. The two are often done together in one sitting to give a fuller picture.
How is the scan done?
- You lie on your back with your right arm raised above your head.
- A little gel is applied and the probe is placed over the right side of your abdomen.
- You may be asked to breathe in and hold briefly while a few readings are taken.
- The scan adds only a few minutes to a routine ultrasound. There are no needles and no radiation.
You may be asked to avoid heavy meals for a few hours beforehand, as this can make the reading more reliable. Our team will tell you if any preparation is needed when you book.
What do the results mean?
In general, a lower stiffness value suggests soft, healthy liver tissue, while higher values suggest more scarring. The exact thresholds depend on the underlying condition and the equipment used, so your report is always interpreted alongside your history, examination and blood tests. Elastography is a guide that helps your treating doctor decide on next steps β it is not a diagnosis on its own.
